We welcome you to our office and are happy to have the opportunity to serve you. We pride ourselves on trying to make dentistry a pleasant experience. Initially we treat any existing dental problems. However, our goal is also prevention - controlling the process of tooth decay and gum disease. This can be achieved through your interest and cooperation, along with our guidance. On this web page you will find our Patient Registration Form, Health History Form and our Notice of Privacy Practice. Also there is a form requesting current dental x-rays from your previous dentist. Please print and fill out this form and mail or fax it to your previous dentist before your visit so that we may have any current films here at the time of your appointment. Please bring any insurance cards or forms for us to complete. We look forward to seeing you. Please call if you have any questions. These files are in PDF format and you will need to print them to complete the fields. You can fax them to the office or bring them with you at your first appointment.
